At Evergreen Nursing and Rehabilitation, we take great pride in ensuring that our residents are as comfortable as possible during their stay.

Physical, occupational and speech therapy are the all important first steps to improved quality of life, and at Evergreen Nursing and Rehabilitation, our residents receive attention and care from qualified professionals. Each resident is evaluated based on his or her functional limitations, and a resident-specific program is implemented to help the resident gain strength, mobility and prevent recurring injury. Additionally, our upbeat staff motivates and educates residents to take care of themselves.
Evergreen Nursing and Rehabilitation is Medicare and Medicaid certified. We also accept private or self-pay residents and most private insurance policies.

Recreation
It is important for residents and staff to have some fun, so our Activities Director ensures that every day includes time to play. From off-site trips to on-site parties, opportunities to “get away” enhance each resident’s progress. For the resident, it means taking a break from the physical demands of recovery. For the staff, it means interacting with residents in a relaxed setting. And for everyone, it’s a chance to recharge for the challenges that lie ahead.

At Evergreen Nursing and Rehabilitation, our mission is focused on healing. Our goal is to assist each of our residents in their journey toward recovery and to see them return to their day-to-day activities. Our facility is equipped to help residents in their transition from hospital to home, offering 24 hour per day skilled nursing services, as well as the occupational, physical and speech therapy necessary to get back to the business of living.
Our Staff
Our employees are committed to excellence because we know how much you care. Our entire staff routinely receives in-service educational training to better meet the needs of our residents. Our skilled nursing staff receives continuing educational training beyond what is required by law so we may keep abreast of ever-changing health care delivery.
