We are committed to providing quality care and improving the quality of life for our residents. We are a 176-bed skilled nursing facility. Our staff of RN’s, LPN’s, and CNA’s, under the direction of the resident’s attending physician, develops an individualized plan of care to meet the need of each and every resident. We stand ready to assist in every possible way from extending an understanding ear to providing your loved one of the finest skilled nursing and rehabilitative services available.
Irwinton Gardens is our Dementia and Alzheimer's Disease Unit. We understand the special needs of individuals diagnosed with Dementia and Alzheimer’s Disease. Our staff has been trained to meet the medical, activity, nutritional, and daily grooming needs of the residents on this unit. Our residents are encouraged to maintain their daily routines and functions as long as possible. We encourage family/sponsor involvement and gladly provide support and education. The unit has semiprivate rooms available. We have a beautiful homelike dayroom/dining room as well as offering a gated courtyard and gazebo for residents and their families to enjoy.
Crowne Health Care of Eufaula is Medicare and Medicaid certified. We also accept private or self-pay residents and most private insurance policies.

Recreational Therapy
Our activities department provides a wide variety of activity programs daily. These programs are varied to meet the interest and capabilities of all residents. This department greatly enhances the quality of life for our residents by providing many fun events, outings and socialization opportunities.
Therapy Services
Should therapy be indicated, Crowne offers physical, occupational, and speech therapy to ensure that each resident reaches and maintains their highest level of function.
Stroke, major joint replacement, general weakness and wounds are just a few of the medical issues addressed. Other less severe disabilities are monitored closely by our restorative nursing team in order to catch and resolve problems at the earliest possible stage.
Many of our rehabilitation patients return home within just a few weeks. That’s a fact that surprises families, but it is one we are extremely proud of. Not so long ago, illnesses and injuries often meant permanent disability for the elderly. Today we understand the benefit of therapy and the value of a senior returning home to their family. We tailor the intensity and duration of therapy based on a resident’s needs. We can provide services 7 days a week.
Social Services
Each nursing unit has a full time social service designee to meet the psycho-social needs of each resident. The social workers offer individual support and counseling as residents adjust to their changing needs and environment.
Dietary
Our dietary staff offers 3 meals daily to meet the nutritional needs of each resident. We offer varieties that meet all resident preferences. We offer snacks for in-between meals. Our full time dietician is available to every resident.
Our Staff
Our employees are committed to excellence because we know how much you care. Our entire staff routinely receives in-service educational training to better meet the needs of our residents. Our skilled nursing staff receives continuing educational training beyond what is required by law so we may keep abreast of ever-changing health care delivery.
